Mesenteric Cyst: Abdominal Lymphangioma

Cisto mesentérico é definido como qualquer lesão cística localizada entre os folhetos do mesentério, do duodeno ao reto, sendo mais comumente encontrado ao nível do íleo. Desde sua primeira descrição em 1507 por Benevienae até 1993 há somente cerca de 820 casos relatados na literatura4,5,7. Linfangiomas são tumores benignos, de origem provavelmente congênita, mais comuns em regiões cervical e axilares. São incomuns em localização abdominal e pancreática1. Estima-se sua incidência em torno de 1:100.000 internamentos em adultos e 1:20.000 em crianças. A primeira excisão foi realizada por Tillaux (apud Chung)5 somente em 1802. Apesar do longo tempo do reconhecimento desta enfermidade, sua origem, classificação e patologia permanecem controversas. A maior incidência encontra-se entre a terceira e quarta décadas de vida, sendo 75% dos casos diagnosticados após os 10 anos de idade, de modo acidental, com discreto predomínio do sexo feminino. O termo linfangioma é utilizado de forma apropriada quando há isolamento hemodinâmico, ou seja, a lesão não está relacionada com o sistema arterial ou venoso10,13. Os linfangiomas constituem um dos grandes grupos dos chamados hamartomas vasculares, que resultam de falha no desenvolvimento evolutivo do sistema vascular, incluindo linfáticos e/ou artérias e veias3. Esses tumores linfáticos são divididos em: 1) simples, com canais linfáticos capilares; 2) cavernosos, com linfáticos dilatados e presença de cápsula; e 3) malformações macrocísticas, clinicamente denominados “higroma cístico”6. Este é o tipo mais comum, e os locais mais acometidos são cabeça e pescoço. O principal diagnóstico diferencial é hemangioma, cisto branquial, lipomas e rabdmiossarcoma. O diagnostico é feito por biópsia de nódulo suspeito, e o principal tratamento utilizado é a excisão cirúrgica.
